Design
Packaging, theme and specs
Nurse Special Patrol has been sitting at the very top of my wishlist for quite some time. Its dual-layer structure, with a firm front and a soft back, is an incredibly rare find in the onahole world. But let’s start with the packaging. The box design definitely caught my eye with the hugely breasted nurse squatting down wearing thigh-high socks and a nurse vest that’s bursting open, struggling to contain her oversized chest.
The color combination of Nurse Special Patrol is striking. You’ve got a beige, firmer front layer and a bright pink, softer back layer. It’s a super rare combo that really stands out, both visually and functionally.
There’s also a concave area in the middle of the toy that improves grip, making it much more comfortable to hold and control during use.
ToysHeart is known for creating some of the most unique and high-quality onaholes on the market. They also make Onatsuyu Lotion, the most popular onahole lube.
Their softest onahole, Mashiro’s Super Soft Hole, is one of my all-time favorites and holds a solid spot in my top 5. If you’re into high-quality, creative toys, definitely check out some of their other standout products like G-19 Bordeaux Soft, Babumi BIG or Mashiro’s Super soft hole.
Nurse Special Patrol is from ToysHeart and is themed after sexy nurse. Measuring 150mm in length with a tunnel length of 135mm, this compact yet satisfying onahole weighs a comfortable 390g. It features a single closed-hole design and is made in Japan with high-quality TPE. It’s double-layered with a very firm beige front layer and a soft pink back layer.

Feeling
Front layer
The front layer of Nurse Special Patrol feels absolutely epic. Unlike most smooth or textured onahole materials, It doesn’t feel smooth but rather creates a pleasant friction when you run your fingers along it, similar to the friction of real skin. The beige color looks very inviting and the opening is very minimalistic with almost no details. You can really feel the individual tiny bumps of the texture inside the front layer.
This outer layer is made with ToysHeart’s ヒトハダ (Hitohada) material, which literally translates to “human skin” in Japanese. It’s designed to mimic the outer feel of real skin, and honestly, it pulls that off really well. It’s also very firm about a “+3” on my softness scale.
Back layer
Once you make it through the firm and tight entrance, Nurse Special Patrol treats you to a completely different sensation: the back layer. This pink section feels much smoother and softer, like a cozy reward waiting deeper inside.
The texture here is very noticeable in a great way. It features three raised lines running lengthwise, with elevated ridges crossing them, creating a dynamic, massaging sensation as you move through it.
The pink material itself is comfortably soft, not super soft, but gentle enough to contrast the firmness of the front very well. I’d rate it a “-1” on my softness scale.
The front is definitely tighter, while the back is stretchier and more relaxed. The suction is good but the texture is the most noticeable part.
This onahole gives you real control over your session. Once you’re deep inside the soft, pink back layer, you can enjoy a relaxed and gentle experience. But as soon as you pull back into the firmer, tighter front layer, the sensation ramps up fast.
By pulling out further and making the firm Hitohada material stroke your full length it becomes really intense. That can be really helpful when the sensation of the soft pink layer starts to fade while you’re edging, to get right back to riding that edge. That way you can control how long or intense your sessions are. I give Nurse Special Patro a “+1” on my intensity scale.

Care
Cleaning:
Gently flip Nurse Special Patrol inside out (but don’t leave it like that for too long), then wash it with soap and water. Once clean, flip it back to its original shape and let it dry completely.
Powdering:
To keep your onahole from getting sticky, apply a light coat of onahole powder or plain cornstarch. Brush off any excess, and it’ll feel just like real skin!
Storing:
Store your toy in a cool, dry place away from direct sunlight. UV rays can damage it, moisture can lead to mold, and heat can warp its shape. Over time, onaholes naturally lose some of their oils, which makes them firmer. This process happens faster if they’re in contact with materials like cotton, paper, or wood, which absorb oil. To minimize this, use a breathable plastic bag or container for storage.
Durability:
The firm front material and stretchy TPE make this onahole quite durable. The circular opening is simple yet sturdy, so tearing shouldn’t be a concern. With proper care, this onahole can last for years!

Rating
Nurse Special Patrol from ToysHeart earns a fabulous 8.5 out of 10. This is one of the most unique dual-layered constructions I’ve tried, firm at the front, soft at the back. That combination is super rare, and it opens up a whole new kind of experience. It’s like having two different toys in one.
If you enjoy stimulation that’s gentle at the glans and firmer along the shaft, this toy is for you. And if you like to mix up intensity during otherwise relaxed sessions, you’ll appreciate how easy it is to switch gears mid-use.
If you stay shallow, the soft back layer gives you a smooth, slow-burn pleasure. But if you use longer stroke, it ramps up the stimulation fast.
The only thing I would’ve loved more is if the soft layer was just a little softer, closer to something like ToysHeart’s Mashiro’s Super Soft Hole. But honestly, the contrast works surprisingly well the way it is.
This is the kind of toy that stands out in my collection, not just for its looks, but for how cleverly it’s designed.
